REGISTRATION INSTRUCTIONS – Hardcopy Forms
Hardcopies of the registration forms are available in the church Narthex or at the church office. Fill out the forms and submit the completed hardcopy forms to the church office during office hours or place the completed forms in the secure mailbox outside the church office during off hours.

Otherwise, complete the fillable registration forms below.
Complete the entries on the forms. To PRINT the forms, go to the “3 dots” at the top right of the form and select “Print”. Submit the completed hardcopies to the church office during office hours or place the completed forms in the secure mailbox outside the church office during off hours.
